Referring to our HCBS Services

Home and Community Based Services (HCBS) are a way for us to provide extra support and skill building, if you are eligible for these services and request them from your Home and Community Based Services (HCBS) are primarily non-clinical, behavioral health rehabilitation interventions. HCBS are considered wrap-around supplemental services which supports a person’s recovery to improve their quality of life and achieve personal goals such as employment and more socialization. As, we know traditional clinical treatment alone usually isn’t enough and there are many elements in a person’s wellness and recovery toolkit. Some people may not be ready for clinical treatment but want and need rehab services which can possibly lead to someone being more engaged in treatment. Rainbow Heights Club is designated to provide four of the available HCBS intervention as listed below.ur health home care manager. We are designated to provide four Home and Community Based Services:

Peer Support – We have 8 NY State certified Peer Specialists on staff, who bring their experience with mental illness and recovery, as well as their experiences as LGBTQ community members. Talk to people who’ve faced challenges like your own, and learn how they overcame them and how you can too.

 

Psychosocial RehabilitationWe can assist you in rebuilding skills such as healthy communication, self-care, socialization, and relationship building. Learn to ride the subway, get around the city, and connect to natural supports like parks, museums, and support groups, to help you stay independent and strong in your recovery.


Habilitation – We can help you build new skills, from making new friends to navigating the City to budgeting, and practice them in a supportive environment.

Family Support –We can help you and your family build acceptance and mutual respect, so that your family can understand your needs and support you.

HCBS is available to adults living with Serious Mental Illness (SMI) and Substance Use Disorder (SUD) in Health and Recovery Plans (HARPs) and/or HIV (Special Needs Plans) SNPs.

We currently work with the following MCOs: Beacon, Empire BCBS, HealthFirst, Fidelis, Optum, and United.

PROVIDERS TO MAKE A REFERRAL

Fax to 718.285.8610 / Attention RHC or email to EddieHardaway@rainbowheights.org a copy of your HCBS eligibility assessment, initial Plan of Care (POC), & the Level of Service Determination (LOSD) letter from the Manage Care Organization (MCO).

We will contact both the referring organization & the client directly to arrange a joint meeting (when possible) at a location of your choice.

Once the LOSD assessment is completed (we are allowed a maximum of three meetings to complete it) we will forward it to the assigned MCO for approval & HCBS care will begin (usually takes three business days to receive approval).

CONSUMERS TO MAKE A REFERRAL

Due to the COVID-19 outbreak, Rainbow Heights Club is temporarily pausing on-site services for members. We are providing support services via telephone from 10 am to 5 pm Monday-Friday.
