Community-Oriented Recovery Empowerment (CORE) services are a way for us to provide extra support and skill building for people eligible for these services. CORE services are considered wrap-around supplemental services which support a person’s recovery to improve their quality of life and achieve personal goals such as employment and more socialization.

 

Rainbow Heights Club is designated to provide three CORE services as listed below:​

Peer Support

We have 8 NY State certified Peer Specialists on staff, who bring their experience with mental illness and recovery, as well as their experiences as LGBTQ community members. Talk to people who’ve faced challenges like your own, and learn how they overcame them and how you can too.

Psychosocial Rehabilitation

We can assist you in rebuilding skills such as healthy communication, self-care, socialization, and relationship building. Learn to ride the subway, get around the city, and connect to natural supports like parks, museums, and support groups, to help you stay independent and strong in your recovery.

We can help you and your family build acceptance and mutual respect, so that your family can understand your needs and support you.

We will contact both the referring organization & the client directly to arrange a joint meeting (when possible) at a location of your choice.

​

Once the Level of Service Determination (LOSD) assessment is completed, we will forward it to the assigned MCO for approval & CORE services will begin.

CORE services are available to adults living with Serious Mental Illness (SMI) and Substance Use Disorder (SUD) who are enrolled in Health and Recovery Plans (HARPs) and/or HIV Special Needs Plans (SNPs).

​

We currently work with the following MCOs: Beacon Health Strategies, Empire BCBS, HealthFirst, Fidelis, MetroPlus Health and United Healthcare.
